Submissions must be made by the original owner of the equipment. The person conducting the interview must submit the form; otherwise, please indicate who is completing the form. Customers must be in Discord and in good standing. The use of third-party supplies will not be eligible for a gift card. This can be during our visit or by reviewing previous purchase histories or tech support history. We are seeking authenticity in the interview; no false information should be provided.
